We love talking about AI transformation. Keynotes, roadmaps, pilots, proof-of-concepts. Big words, bold ambitions. Every conference has a slot for it. Every board agenda has a line item. And yet, for all the noise we make about AI strategy, the most consequential shift isn’t happening in the strategy decks. It’s happening in the background, in the workflows nobody is formally reviewing, in the small moments where a human decides, just this once, to let the system handle it.
The real transformation is already underway. Quietly. Without a sign-off. And most organizations won’t realize it until they’re already well past the point of easy course correction.
It Happened to Us. Probably to You Too.
I’ll give you a personal example from my work. We built a review agent for our commercial content, such as proposals, offerings, and market positioning. It started as a helpful second opinion. A sanity check before the content went out. Clearly in a supporting role, clearly under human control.
Then it became the reference. Then the gatekeeper. And now? The system decides when content goes out. Not because we designed it that way. Not because someone wrote that into a policy. It just happened, one convenient shortcut at a time, until the shortcut became the process. I’m betting we’re not alone. The question is whether your organization has noticed yet.
The Signal You Should Be Watching
CIO Magazine recently published a sharp piece on five signals that tell you when AI has crossed from tool to actor inside your workflows. It’s based on research from Forrester and interviews with technology leaders at Trimble, Cisco, and Phison Electronics, and it’s worth reading in full. However, one signal hit me the hardest.
Your team stops asking “what prompt did you use?” and starts asking “why did the system decide that?”
That’s the moment. The question changes, and so has the dynamic. Aviad Almagor, VP of Technology Innovation at Trimble, puts it plainly: the line is crossed when AI stops answering questions and starts taking actions. In the early phases, systems recommend next steps. Once they start executing those steps, the workflow has fundamentally changed, whether leadership has acknowledged that or not.
Cisco’s Nik Kale describes the same pattern playing out at scale. Initially, humans review AI outputs before they reach customers. Over time, as confidence grows, that review becomes a rubber stamp. Eventually, humans are only involved after something goes wrong. His framing is blunt and worth remembering: the moment humans move from the decision loop to the post-mortem loop, you’ve crossed the threshold. That’s not an efficiency gain. That’s a governance problem that hasn’t surfaced yet.
AI Advancement Without Human Readiness Is Just Expensive Chaos
Forrester predicts that by the end of 2026, CIOs will be forced to decide how far workflows can operate without humans. The uncomfortable truth is that many organizations are already drifting toward that answer — they just haven’t made the decision consciously. Forrester VP Linda Ivy-Rosser points to a recurring pattern: CIOs deploy AI to fix messy, non-standardized processes under pressure, bypassing the hard pre-work of defining decision rights, escalation models, and accountability structures. The result is operational risk that accumulates silently, not because the AI fails, but because governance never kept pace.
The winners of the next phase won’t be the organizations that deployed fastest. They’ll be the ones who answered three questions before autonomy walked in through the back door: Who owns the decision? Who owns the outcome? And who owns it when something goes wrong? AI doesn’t remove accountability. It just makes the absence of it much more painful and much more public.
The Operating Model Has Already Changed, Did You Notice?
Here’s the part that gets less attention than it deserves. When AI moves from assisting individuals to orchestrating end-to-end workflows, it doesn’t just change how work gets done; it changes what work even means. At Trimble (a tech company), Almagor describes a shift from role-based execution to outcome-driven workflows. Instead of AI tools supporting schedulers or planners independently, agentic systems now monitor conditions and adjust plans continuously across the entire chain. The roles don’t disappear, but they stop being the organizing principle.
Forrester’s Ivy-Rosser adds another uncomfortable observation: many organizations have handed over process complexity to vendors through managed services without shifting to outcome-driven contracts. The result is that vendors end up making strategic decisions because the enterprise never clarified where utility ends, and competitive advantage begins. That’s not a technology problem. That’s a leadership clarity problem, and AI is simply the latest force that exposes it.
When work reorganizes itself around outcomes instead of roles, the operating model has changed. Not because someone decided it. Because the technology quietly made it so. That’s not always bad. But it needs to be deliberate. Set up your decision rights before autonomy negotiates them for you.
Culture Is an Operational Control, Treat It Like One
One of the less obvious signals from the CIO Magazine piece is cultural, and it’s the one I think gets skipped most often in the governance conversation. Organizations that are ready for higher levels of AI autonomy share a specific behavioral trait: they’re comfortable with probabilistic outcomes. They don’t expect deterministic answers from AI systems. They treat uncertainty as an input, not a failure, and they design human-in-the-loop mechanisms accordingly.
The danger runs in both directions. Under-trust means your teams constantly second-guess the system and you get none of the efficiency gains you were promised. Over-trust, and this is the more common failure mode, means humans disengage once AI performance stabilizes, even as the blast radius of decisions quietly grows. Kale describes this as a silent erosion of vigilance that often precedes governance crises. Nobody decided to stop paying attention. It just became easier not to.
Technical readiness without behavioral readiness is a leading indicator of failure. Retrain your managers to supervise digital workers, not just consume tool outputs. That’s a different skill set, and most organizations haven’t started building it yet.

The shift from assistive AI to agentic AI is not a future event. It’s a present condition in most organizations doing serious AI work, including ours. The question isn’t whether it’s happening. The question is whether you’re shaping it or inheriting it.
